Immunizations for the 2025-2026 School Year
All incoming preschoolers are required to have a physical along with the following:
4 doses of DTaP
3 doses of Polio
3 doses Hep B
1 dose of MMR (after 1st Birthday)
3 or 4 doses of HIB
1 dose of Varicella (after 1st Birthday)
2 doses of Hep A
3 or 4 doses of Prenvar
Annual Flu vaccine
All incoming kindergartners are required to have their "kindergarten shots" which include the following:
5 doses of DTaP
4 doses of Polio
2 doses of MMR
3 doses of Hepatitis
2 doses of Varicella (chickenpox)
All incoming 7th graders are required to have the following:
Tetanus booster (Tdap)
Meningococcal vaccine
All incoming 12th graders are required to have the following:
Meningococcal vaccine (You must be over the age of 16 in order for this to count as your senior required immunization.)

The Crawford County Health Department offers clinics year-round on Thursdays (except Thanksgiving and other Holidays) with at least two evening options each month. Please remember to take your insurance card as well as your shot record to allow them to update it for you. They are booking appointments. Call (419)562-5871 to get on the schedule.
